Abstrakt
Sagedamad intraoperatiivsed tüsistused on hüpotensioon, ebanormaalne kehatemeratuur (hüpotermia ja hüpertermia), ebanormaalne südame löögisagedus (tahhükardia ja bradükardia), rütmihäired ja hüpoventilatsioon. Tüsistused on sageli seotud kasutatud ainete, protseduuri iseloomu ja looma terviseseisundiga. Võimalike tüsistuste avastamiseks tuleb looma protseduuri ajal jälgida.
Kirurgiliste menetlustega kaasneb valu. Anesteseeritud loomal väljendub valu hemodünaamilise ebastabiilsusena (südame löögisageduse kiirenemine ja vererõhu tõus). Mitme kirurgilise menetluse puhul soovitatakse kasutada multimodaalset analgeesiat. Notsitseptsiooni blokeerimiseks kasutatakse preventiivset analgeesiat.
Antud lõputöö raames on uuritud ovariohüsterektoomia jooksul tekkinud intraoperatiivset vistseraalset valu, sellega toimetuleku võimalusi, anesteetilist lähenemist ja anesteesiast tingitud tüsistuste esinemissagedust tervetel emastel kassidel. Munasarja-kandesideme ligeerimine on ovariohüsterektoomia kõige valulikum moment. Uuritud kirjanduse kohaselt kasutatakse optimaalse intraoperatiivse analgeesia saavutamiseks opioide (butorfanool, buprenorfiin, metadoon), mida saab kombineerida mittesteroidsete põletikuvastaste ainetega. Ka mõned teised raviained (medetomidiin, ketamiin) on analgeetilise toimega. Munasarja-kandesideme venitamisel ja ligeerimisel on lisaanalgeesia tagamiseks võimalik kasutada lokaalanesteetikume (pinnaanesteesia lidokaiiniga) ning opioide (fentanüül). Tänapäeval on palju uuritud ka maropitandi analgeetilist toimet.
Lõputöö tulemusena selgus, et opioidi (butorfanool) kasutamise korral esineb vähem valu. Munasarja-kandesideme ligeerimisel on täheldatud südame löögisageduse kiirenemist ja vererõhu tõusu. Kõige sagedasem tüsistus on hüpotermia, mida on esinenud teatud ajahetkedel enamikul kassidel.
The most frequent intraoperative complications are hypotension, core temperature changes (hypothermia and hyperthermia), heart rate changes (tachycardia and bradycardia), dysrhythmia and hypoventilation. Complications are frequently associated with the used drugs, type of procedure and health condition of an animal. Patient monitoring is necessary to find out possible complications. Surgical procedures are accompanied by pain. Pain in animal in anesthesia is expressed as hemodynamic instability (increase in heart rate and blood pressure). Multimodal analgesia is the approach of choice in majority of surgical procedures. Preventive analgesia is an opportunity to block nociception. The aim of this study is to analyze intraoperative visceral pain during ovariohysterectomy, its treatment modalities, anesthetic approach and anesthetic complications frequency in healthy female cats. Ligation of suspensory ligament of ovary is the most painful moment. Accordingly to studied literature, intraoperative analgesia of choice includes opioids (butorphanol, buprenorphine, methadone) and non-steroidal anti-inflammatory drugs.Some other have analgesic properties (medetomidine, ketamine). During the ligation of suspensory ligament of ovary to provide additional analgesia local anesthetics (lidocaine “splash”) and opioids (fentanyl) can be used. A lot of researches are also providing information about maropitant analgesic effect. As a result of this study, author found out that there is less pain if opioid (butorphanol) is used. During the ligation of suspensory ligament of ovary, increase in heart rate and mean blood pressure was detected.
